EAR RECONSTRUCTION SURGERY

Ear reconstruction surgery is one of the most challenging operations to perform, due to the complex shape that must be created for the ear to look natural. Only highly skilled & experienced Microtia surgeons can perform the surgery to get consistent results.

RIB CARTILAGE EAR SURGERY

It is one of the most commonly used techniques for ear reconstruction. The surgery involves removing a portion of the patient’s rib cartilage and carving it to create a framework for a new ear. This framework is then placed in a pocket under the scalp skin. Rib Cartilage ear surgery might need 2-4 surgeries for the optimum results, but highly skilled surgeons are able to achieve optimum results in least number of surgeries.

RIB CARTILAGE EAR SURGERY

This surgery is age dependent and the patient needs to be at least8 years of age, as the surgery cannot be carried out without the patient having developed sufficient rib cartilage. The surgery might be delayed if there is not enough cartilage at their disposal to create a symmetric ear. Once the chest circumference of child reaches 60cm, irrespective of the age, we can perform the surgery.

Procedure

The basic principle of Rib Cartilage Ear Surgery is to reconstruct the contours of a normal ear. As the ear is complex in shape, a firm supporting framework has to be used in order for these contours to be properly reproduced.

The procedure involves harvesting the rib cartilage from the patient. This leaves a scar of some 6-8 cms in length but it does not deform the patient’s chest, nor does it hamper the playing of sports.

The new ear is sculpted from the patient’s own cartilage and skin and therefore is alive, grows with a child, and heals without problems. This allows a person to resume normal sports, swimming, and PE activities 4-6 weeks after surgery with no particular concerns.

FIRST OPERATION

During the first operation, the rib cartilage is harvested and a framework is prepared that is then implanted beneath the skin. In this stage of the surgery, all the contours of the ear will be reproduced, but there will be no space behind the ear. The ear appears normal from the side profile but will not stand out like the other one.

SECOND OPERATION

To make the ear look symmetrical to the other ear, second operation is done to create post-auricular sulcus (space behind the ear) that makes sure that both ears matches when they are viewed face-on.

The interval to be observed between these two operations ranges from three to six months, depending on the particular patient’s situation.

PROMINENT EARS & TREATMENT

Prominent ear, also known as otapostasis or bat ear is an abnormally protruding human ear. In this condition, the concha (the depression in the external ear leading to its central opening) is large with poorly developed antihelix (a curved prominence of cartilage parallel with and in front of the helix on the pinna) and scapha (the curved depression separating the helix and antihelix of the ear).

Approximately 1-2 % of the total population is affected by prominent ears problem. It is an inherited problem and may be unilateral or bilateral and could be a result of lack or malformation of cartilage during primitive ear development. 30% of babies who have prominent ears are born with normal ears. It is only after 3 months that the problem starts to arise and can get worse when the soft cartilage is repeatedly bent over during breast-feeding.

Causes of protrusion of the external ear:

1. Underdevelopment or effacement of the antihelix
2. Overdevelopment of the deep concha, or
3. A combination of both of these features
Even though prominent ears don’t have any functioning issues, it could cause psychological problem in the child & can lead to reduced self-esteem & poor performance in school.

OTOPLASTY PROCEDURE

Otoplasty is a surgical treatment for correcting the prominent ears. It can be carried out in the child from the age of 8 years & above. The surgery can also be carried out in adult patients.

In prominent or Protruding ears, the antihelix is not properly formed or is completely missing. To correct this, antihelix has to be made.

Some patients have prominent ear of a smaller grade, in which the prominent ear has the antihelix form but that antihelix at the middle portion is not more prominent. So to reconstruct that anti helix by suturing method is performed.

Otoplasty is usually done under a general anesthesia in children & local anesthesia in adults. Once the patient is anesthetized, an incision is made behind the patient’s ear in the fold of the skin where the ear meets the head. Through this incision, extra skin is excised. After this, Concha is sutured with mastoid (bone behind the ear), its called concha mastoid sutures, so the ear sticks towards the back.

Each patient is different for prominent ear. In some patients the inner part of antihelix, the concha becomes bigger which then needs to be removed. In some patients the angle of the fold in the ear cartilage may cause the ear to protrude, or in some the ear lobe may be unusually large.
